To become a new patient at Tri-State Neurosurgery, please contact our office at (318) 212-8176, or have your physician fax a referral request to (318) 212-8186.

Our Location

We are located at 2551 Greenwood Road, Suite 320. We are Conveniently located on the campus of Willis-Knighton Medical Center (North).

Required Patient Forms

Save time by printing and completing your new patient information packet before your visit. Other signature paperwork will need your signature but must be competed the day of your appointment in our office.

What to Bring to Your Appointment

  • Your insurance card and any authorization your insurance has sent you
  • All Required Forms Listed Above (please fill out prior to your appointment)
  • List of questions you may have
  • CD disc of X-ray, CT or MRI scans related to your condition. You may need to obtain the CD disc from the radiology or medical records department of the hospital where you underwent the imaging. This process may take a few days so please plan ahead and bring your CD with you to our appointment. Failure to do so may delay our decision-making regarding your care.
  • Paper and pen to take notes

Fees & Billing

All Co-pays and Deductibles are due at the time of your visit.

We ask that you provide your current insurance information at every visit, to make sure we have your most up to date insurance information

Insurance

Tri-State Neurosurgery aaccepts most insurance plans. Your insurance coverage is an agreement between you and the insurance provider . We are required to collect all Co-pays and deductibles that apply at the time of service.

We ask that you provide us with a current ID along with your insurance cards. As always the final responsibility of the clinic charges are the patients responsibility. Failure to keep us informed of any changes in coverage that may occur with your current insurance coverage could result in full patient responsibility.

This will help us be more efficient as we provide the best care for you.

Prescriptions

We do not provide narcotic pain medications for brain or spine disorders, unless you have undergone surgery by one of our surgeons. We do not routinely provide anti-anxiety medications, including Ativan, Xanax, or Valium. We do provide referrals to pain management specialists for appropriate patients.

If you have had surgery by one of our surgeons, then you will receive a prescription for any necessary medications at your hospital discharge. Narcotic pain medications may be prescribed for up to 3 months after your surgery depending on the need. Re-fills of narcotic pain medications will be given when you visit us in person in the office. Due to federal and local regulations, we are unable to mail or call-in narcotic pain medications to your home or pharmacy.

If you need refills of non-narcotic medications, including muscle relaxers, stool softeners, or nerve medications, please call our prescription line at 318-212-8176. Leave your name, date-of-birth, your phone number, your pharmacy, and your pharmacy’s location and phone number. We will call your medication into your preferred pharmacy for pick-up within 48 hours.

Follow-up Appointment

After your initial new patient assessment, you will be given a follow-up appointment if needed during your check out at our front desk.

After your surgery, your follow-up appointments will be listed on your discharge paperwork. In addition, our nurse will call you within 48 hours of your discharge to see how you are doing and to confirm your follow-up appointments.

If you are a established patient and we are following you on a regular basis, then you will receive a letter reminding you of your next follow-up in the mail. If you need to be seen sooner or have any questions, you may call our office at 318-212-8176.

Release of Information

Your information is privacy-protected. You must sign a request in order for us to release information to your insurance company or to friends / family.
