What Type of Referral Is This?

Patient Self-Referral

If you are a patient with Parkinson's disease, dystonia, or tremors and would like to be seen but are having difficulty obtaining a referral from a primary care physician or a neurologist, please call our office at (318) 212-8176 for more information on self-referrals. We will do our best to accommodate you and your family members.

Physician Referral

If you are a referring physician, please fax the referral informtion form to (318) 212-8186 or mail the patient’s relevant medical records, including brain MRI films (hardcopy or disk) if done outside of the WK system, as soon as the referral is made to assist in our care of the patient.

Reasons To Refer a Patient To Our Surgical Movement Disorder Clinic

Patient has one of these diagnoses:

  • Parkinson’s disease
  • Essential tremor
  • Primary Dystonia

AND...

  • Medication/Botox is no longer helping
  • Medication benefit does not last
    • Wearing off or Unpredictable off times
    • "My meds only work for 1 or 2 hours"
    • "My meds suddenly stop working"
  • Meds cause abnormal movements
    • Dyskinesias
  • Medication is not tolerated
    • Due to nausea or other side effects
